Cytokinetics announced positive top-line results from the pivotal ACACIA-HCM Phase 3 trial of aficamten in non-obstructive hypertrophic cardiomyopathy (nHCM), meeting both dual primary endpoints: KCCQ-CSS and pVO2. The data were presented at ESC 2026 and published in NEJM, and the company plans to submit a supplemental NDA to the FDA in Q4 2026. This success validates aficamten's efficacy in nHCM, a patient population with no approved myosin inhibitor after Bristol Myers Squibb's Camzyos failed in ODYSSEY-HCM. The news directly addresses a key risk factor in our prior thesis, which had flagged ACACIA failure as a potential bear-case trigger. However, the company still faces significant launch execution challenges for Myqorzo in oHCM and a leveraged balance sheet, so the positive trial result is necessary but not sufficient to justify current valuation.
Implication
The ACACIA-HCM success removes a major overhang and opens a large nHCM market, potentially supporting the bull case value of $85 per share. However, management's ability to execute commercially remains unproven, and the balance sheet remains strained with negative equity and high burn. The supplemental NDA submission in Q4 provides a near-term catalyst, but FDA approval and subsequent launch dynamics in nHCM will take time. Given the stock's recent run and crowded bullish positioning, a significant portion of this news may already be priced in. Thesis delta
The positive ACACIA-HCM results increase the probability of the bull case and reduce the bear case which hinged on nHCM failure. Our previous 'POTENTIAL SELL' rating was partly based on the risk of ACACIA failure; with that risk now resolved positively, our stance shifts to 'HOLD' pending further commercial data. We are not yet upgrading to buy because valuation still embeds aggressive assumptions for Myqorzo's oHCM launch and nHCM market capture.
